If you injure yourself on the job, please have the claim filed to the Business office within 3 business days from the time of your injury.  First report of injury forms can be found with each building secretary, and also with Dee at the Business office.

If you seek medical treatment, please inform the medical facility that your injury is workers' compensation and all requests for payment should be forwarded to the Douglas School District Business Office - Attention: Dee Waltman.
